I would need more information than what you’ve provided, such as how long it has been planted in this location, what is the soil like, and how long has it been showing these signs, and did it happen all at once, or in stages. Without closer pictures to see what’s happening, it’s impossible for me to give you any advice, other than to get an arborist (tree specialist) in to look at it in person.
The other thing you could do is take pictures and a piece of the tree to your local plant nursery and see if there is a horticulturist there that could help.
Or, you could just dig the tree up and get rid of the sickly thing, and replace it with something else. Don’t plant another maple in the same place in case it’s something in the soil that will also affect another tree of the same genus.
